"What's the worst that could happen?" worksheet

  Hello all, I'm new to the forum.  I've been going through a tough time lately with depression and anxiety; all I seem to want to do is stay in bed and not do much of anything. I have a psychiatrist I go to, but we don't really talk much.  He's a good guy, but all he really seems to do is knod his head to me when we talk and give cheery little phrases. I want to get better but am having trouble. I'm on a combination perscription that includes cylexa and brupropion. It helped elevate my mood from where it was, but It seems like I'm just closing off doors to the life I used to have before this problem struck me. I feel like I obsess over certain thoughts. Being like this makes me not want to talk with friends or go out much.

  I Once had a worksheet I got from another psychologist that helped me.  It had about 15 questions on it I believe, with questions like;

  "What's the best that could happen?"
  "What's the worst that could happen?"
  "How strong is the feeeling?"
  "How strong is the feeling now?"

    etc

  Does anyone know where I could find a copy?

Before anyone can give you a good enough reply perhaps a bit more information would be useful...

I was wondering what gender you are, and how old you are?

Are cylexa and brupropion both anti depressants?
